WHICH portal do you wish to access?

Wayne Memorial Hospital is different from Wayne Memorial Community Health Centers (WMCHC), which is an independent affiliate of the hospital and operates many physician offices including primary care, women’s health, and behavioral health.

The Patient Portal for SPECIALTY OFFICES including cardiology, gastroenterology, nephrology, neurology, pulmonary, urology, urogynecology and surgery has a separate link.

DO I NEED MORE THAN ONE PORTAL ACCOUNT? 

The Hospital maintains different data sets from WMCHC practices and uses different software. Therefore, it is necessary for you to have both myWMH account as well as a CHCconnect  account if you utilize both our WMCHC physician offices and the hospital.

  

Wayne Memorial Hospital’s portal.

It gives you confidential access to your medical records at Wayne Memorial Hospital, such as X-rays and lab (blood test, urinalysis, etc.) results. With myWMH you can view your medical information, manage your appointments and your billing information. Wayne Memorial Community Health Centers portal.

This covers primarily our affiliate and physician offices—primary care, women’s health, and behavioral health. With CHCconnect, you can request appointments, prescription renewals, view office visit summaries, ask questions of staff and providers, ask billing questions and pay online. You can also ask for your test results if, for example, you haven’t seen them on your myWMH portal page or they were not done at Wayne Memorial.          Wayne Memorial Community Health Centers’ Pediatrics portal.

This is the former Pediatric Practices of NEPA site and covers its Honesdale, Lords Valley, Sterling and Waymart offices. This site allows parents and guardians to access their child’s record from their home computer or smart phone and send or receive secure messages from our providers. FREQUENTLY ASKED QUESTIONS

Why are my medications different on the different portals?

The information in each portal is as current as your last visit to that organization.  It is possible for your medications within the myWMH portal and the CHCconnect to be different. However, your information will be updated with your next visit with a nurse or doctor.

What if information is wrong on my health history or other area?

Depending on the information, you may either change it on the site itself (as in My Account/Profile for CHCconnect) or submit questions. On CHCconnect, you can click Ask A Question and on myWMH, you can click on Contact Us at the bottom of the Home screen or you can also email us at portalmessages@wmh.org.

Note: Contact Us is for non-medical messaging. Non-medical messages are intended for billing questions, requests for copies of medical records, portal feedback and technical questions related to the portal. PLEASE DO NOT SEND MEDICAL QUESTIONS VIA THE PORTAL. All medical related questions should be addressed with your physician.
