2023 Wayne Memorial Health Foundation Community Grants Available

(Honesdale, PA, May 11, 2023) … Non-profit groups providing health-related programs and in need of funding in the Wayne Memorial service area may qualify for a Community Grant from the Wayne Memorial Health Foundation (WMHF). Applications will be accepted from organizations whose focus includes health care-related services for injuries, disease prevention and treatment, and the promotion of good health and wellbeing for residents of Wayne and Pike Counties, the Greater Carbondale region of Lackawanna County and Forest City in Susquehanna County.

Applicants must complete a WMHF program application that includes proof of their IRS 501(c)3 tax-exempt status; submission of their group’s non-discrimination policy; demonstration that they are not providing unnecessary duplication of services in the community; provision of a written statement of their objectives, target population, anticipated benefits, activities and evaluation procedures.

Applications and guidelines may be downloaded from Wayne Memorial Hospital’s website, www.wmh.org  and clicking on the Community tab or requested by emailing kneier@wmh.org , calling (570) 253-8422 or by writing to Wayne Memorial Hospital, 601 Park Street, Honesdale, PA 18431, Attention: Carol Kneier. Completed applications are due no later than June 23, 2023 via email or hand delivered to the above address.
