The Outreach and Enrollment Coordinators are here to help you find the programs right for you. Coordinators will also help you with completing the applications. Call us!
No one should go without healthcare because they are not able to pay. WMCHC’s goal is to make sure everyone gets the best health care. That’s why we offer the Sliding Fee Discount Program (SFDP) to help you afford the cost of your care at our offices. This can also include a Prescription Discount Drug Card (PDDC) to help you afford the medicines that keep you healthy and/or the Family Planning Discount at our Women’s Health Offices.
The PDDC is a program that helps eligible WMCHC patients get discounts on prescription medications to stay healthy. To be eligible, WMCHC patients have to meet the income guidelines AND explore all resources for medication discounts with our Outreach and Enrollment Coordinators. The medication prescription has to be written by a WMCHC provider.
These programs are based on your household income and are available to all patients of Wayne Memorial Community Health Centers.

You can apply anytime! Coverage will begin immediately after approval and continue until the next open enrollment period (April 1st to July 1st). Everyone must reapply yearly during open enrollment. If you do not reapply your coverage will end on July 1st.
Yes, we will not delay your care while you apply for Sliding Fee. Once you are approved, we will apply the discounts to any balance in the last 90 days. If you are not approved, these balances will need to be paid by you.
There are many ways! You may access an application by clicking Sliding Fee Discount / Prescription Discount Drug Card Program Application.
After completing the application, you may drop it off at any of our WMCHC locations, mail to: WMCHC Outreach and Enrollment Coordinators, 630 Park St., Honesdale, PA 18431; email to: outreach@wmh.org, or fax to: 570-251-6217
If a signed application and proof of income are not received within 90 days, a new application will need to be completed.
We will review the application within 10 business days of receiving all required documents. If you are approved for the Sliding Fee Discount Program or the Prescription Discount Drug Program, you will receive a card and a letter in the mail explaining the program and your costs.
The Sliding Fee Discount Program can be used at all sites listed below:
If you are approved, you will be given a card to use at one of the approved pharmacies. You will need to give this card to the pharmacy to receive the discount.
Approved Pharmacies: Brundage’s Pharmacy (Waymart), Stephen’s Pharmacy (Honesdale) or Red Cross Pharmacy (Forest City).
